Second form of identification

For example, a valid driving licence, birth certificate or marriage certificate.

Back of driving licence (if applicable)

If sending your driving licence as a second form of identification please also upload a photo of the back.

Change of name document (if applicable)

For example, a marriage certificate or deed poll.

Proof of National Insurance number

For example, a recent payslip, HMRC letter, P45 or P60.

Enhanced child workforce only DBS if you have one

Please upload your enhanced child-only DBS certificate. We are only permitted to carry out checks on child workforce DBS certificates and cannot accept adult and child DBS certificates. If you're unsure which certificate you have, please upload it and our team can check it for you. If you don't have one, we can arrange this for you.

International police check (if applicable)

If you have lived or worked abroad for six months or more within the last five years, please upload a letter of good conduct or police check from that country.

Proof of address (dated within the last 3 months)

For example, a utility bill or bank statement. Please upload a paper copy, as we cannot accept online copies.

Qualification certificates

Please upload your highest qualification certificate or any relevant training certificates, such as QTS, a degree or TA training.

Photo

Please upload a recent, clear head and shoulders photo of yourself. This doesn't need to be a passport-style photograph but should be suitable for professional use.
